SDMI Modality Upgrade / Update Questionnaire
We've been informed that you are soon going to update the hardware / software of a modality currently in place at SDMI. To avoid any issues during and after the modality updates, we require that all vendors fill out the following questionnaire. This will help us understand and prepare for any issues that affects the modality or our PACS system after the update / upgrade.
